File Release Policy
All client requests for a copy of their file must be in writing. Once it has been determined that such a release of records is proper, Columbine Counseling Center will take a minimum of five (5) business days to produce such records as we are a small office. There will be no exceptions to this policy.

Court Testimony Policy
The following policy is applicable when any clinical staff member of the Columbine Counseling Center, P.C., is requested to appear at a deposition or in court for the purpose of presenting expert testimony:

Psychological Test Data Policy
It is clear from statute that parties to a Child and Family Investigation (CFI) or a Parental Responsibility Evaluation (PRE) have a right to request the underlying file data. This becomes a problem for psychologists in regards to psychological test data. The American Psychological Association (APA) “Ethical Guidelines for Psychologists” (Rev. 2003) states that psychologists do not promote the use of psychological assessment techniques by unqualified persons. Psychologists conducting court related evaluations are therefore faced with a dilemma when they receive a request for underlying data collected during a CFI or PRE. On the one hand, the statute gives clients the right to the data. Yet psychologists are under ethical constraints to protect clients from harm or misuse of test data.

Welcome to Columbine Counseling Center, P.C. The following information tells you about my services and policies. Our office policy is casual attire. Thank you for selecting me to serve you.

Bill J. Fyfe, Ed.D., Psychologist

Emergency services are not provided for consultation or evaluation clients. I will return calls during normal business hours.

For therapy clients, coverage for emergencies is available at all times. If you have an emergency during off-hours, call our office and follow the instruction given on my voice mail. Fees for emergency care are charged at my regular rate. You may also call West Pines at Exempla Medical Center (303-467-4000) for emergency assistance.

Your file is a confidential record of services. When you conclude work with me, your file remains in storage for seven years. Once in storage, a minimum $100 fee, depending on the size of the file, will be charged for file retrieval except when returning to see me for further treatment.

Support staff is generally available from 9:00 a.m. until 12:00 p.m., Monday through Friday. At other times, the phones are transferred to voice mail. I will make every effort to return calls each day in a timely manner.

Failure to keep an appointment without notifying us will result in a charge of one-half fee for cancellations with less than 24 hours notice, and full fee for missed appointments without notice. (Voice mail messaging is available for cancellations after office hours or on the weekends.)

CCC no longer accepts insurance. It is the responsibility of the parent/guardian to pay for a child’s treatment. If a parent expects a third party to pay some or the entire fee, he or she must deal directly with that person.
